When reacting from IOS app then getting Question mark icon instead of actual reaction icon.
When i click on question mark getting a popup with Error
Error Message: “Add reaction failed”

Could you please provide the iOS SDK + UIKit versions you are using in your application, along with a screenshot of the issue? By the way, are you using any custom emojis?
